The NHTD (Nursing Home Transition & Diversion) Waiver and the TBI (Traumatic Brain Injury) Waiver help seniors and individuals with disabilities receive the right level of care in the community instead of in a nursing facility.
Through these programs, Medna provides and coordinates:
- Service Coordination: Your dedicated Service Coordinator is your main point of contact, guiding you through:
- Initial Service Plans (ISP) – A customized care plan tailored to your needs.
- Ongoing Care Management – Continuous support and adjustments as your situation changes.

Independent Living Skills Training (ILST) – Skill-building to help individuals live more independently.
Community Integration Counseling (CIC) – Emotional and social support for adjusting to community living.

Moving Assistance – Support with moving expenses when transitioning to independent living in the community.
